Jennifer Qadir is a nationally certified Physician Assistant with over 15 years of experience practicing Internal Medicine, caring for seniors and adults with chronic and complex medical conditions. She takes a holistic, whole-health approach to patient care, focusing on overall wellness, nutrition, mobility, and lifestyle factors that impact healing and quality of life.
Jennifer is also certified in wound care (WCC) and has specialized training in advanced wound management. She integrates evidence-based wound treatments with comprehensive medical and wellness support to help patients heal safely and comfortably at home.

Dr. Sowande Buckmire is a board-certified Internal Medicine physician with extensive experience caring for medically complex adults. He earned his medical degree from the American University of Antigua College of Medicine and completed his Internal Medicine residency at the Cleveland Clinic, one of the nation’s top academic medical institutions.
Dr. Buckmire has a strong background in chronic disease management, preventive medicine, and evidence-based care. His experience in high-acuity clinical settings has shaped his commitment to patient safety, thorough evaluation, and collaborative care.
